Program Coordinator
Full Time - Hourly Range: $17 - $25
Day Shift (Monday - Friday) - Weekends as needed - Evenings as needed

The Program Coordinator provides family-driven case management planning, facilitation, and support to individuals and families through assistance with obtaining and sustaining safe, decent and affordable housing.
Key Responsibilities/Essential Functions
Case Management
Assist families in emergency situations and provide crisis intervention when necessary
Conduct in-home, emergency shelter, or in-office visits to identify needs and goals for each family
Conduct housing needs assessments in order to identify and address barriers to housing stability
In partnership with clients, develop, review and amend efficient and effective client case plans
Coordinate client referrals, as appropriate, to community resources and to internal resources
Assist clients with state, federal and local assistance applications
Attend and actively participate in interagency staff and multidisciplinary meetings
Database/ File Maintenance
Maintain appropriate and sufficient documentation to facilitate the delivery of services
Accurately record case notes, follow-ups, update case plans and recertify as required
Input data in a timely manner (48 hours)
Enter North Dakota Continuum of Care (CoC) Program information into the Homeless Management Information System (HMIS) in a timely manner
Legal Aspects
Maintain a high level of integrity and confidentiality of all program participants, documents and records
Adhere to NASW Code of Ethics
Maintain participant and CoC Program documentation, including all financial and programmatic records, for a period of at least five years
Professionals having knowledge of or reasonable cause to suspect that a child is abused or neglected, or has died as a result of abuse or neglect, must notify their supervisor and make a report of the circumstances immediately
Administrative
Collaborate with Director of Family Services to develop policies and procedures for United Way programming
Collaborate with Director of Family Services to evaluate the efficiency of United Way programs
Participate in weekly department meetings
Resource awareness and networking – actively participate in a variety of community meetings with key stakeholders and service providers
Report required statistics on a regular basis
Requirements
Two years of related work experience preferred
Must be people-oriented and be able to communicate effectively in face-to-face situations.
Must have a high level of interpersonal skills to handle sensitive and confidential situations. Position continually requires demonstrated poise, tact and diplomacy.
Must be able to interact and communicate with individuals at all levels of the organization.
Must possess the ability to maintain effective working relationships with others, including other agencies, departments within the organization, and the general public.
Must have the ability to work independently with minimum supervision.
Must be proficient in using the organization’s management software, Microsoft Office products, telephone system and various audio/visual technologies.
